Phlebotomy Blood Draw

Service Description

A phlebotomist will perform a single blood draw appointment. This service is typically used when a provider requests that blood work be collected for testing at an outside laboratory. The patient is responsible for providing the laboratory requisition form from the ordering provider, the collection vials/tubes required for the blood sample, and the pre-labeled shipping packaging needed to send the specimen to the testing laboratory. PRICE INCLUDES: 1 Phlebotomy Blood Draw- up to 2 tubes drawn. ***For additional tubes, centrifugation, refrigeration for multiple days being drawn - additional fees may occur. *Please call 412-952-6524 to discuss any additional questions. Running late? A 15 minute grace period is provided. If the arrival is BEYOND 15 minutes of the scheduled appointment, customer will need to pay for another appointment in full.
